Carrier Setup Checklist

Please ensure you have the following four documents to start the Carrier Setup. They are all essential.

  1. Motor Carrier Authority
  2. W9 Form (Request for Taxpayer Identification Number and Certification)
  3. Certificate Of Insurance
  4. Notice Of Assignment / Voided Check

Together, these four essential documents which are Motor Carrier Authority, W-9 Form, Certificate of Insurance, and Notice of Assignment/Voided Check form the foundation for onboarding and managing carrier relationships in a trucking dispatch business.
